Parent / Guardian Agreement

Required before a student membership begins

The Student Pass is for guests under 18, so a parent or legal guardian needs to read and complete this agreement before the membership can start. It takes about three minutes.

  1. Read the agreement below.
  2. Complete and sign the form at the foot of this page.
  3. Continue to payment — the membership starts from there. A copy is emailed to you straight away.

What the activities involve

“Activities” means use of Avanto Park City’s dry sauna, cold-plunge pools and related facilities, amenities, changing and shower areas, and supervised transitions between them. The sauna may be approximately 195–200°F. Cold-plunge water may be approximately 40–55°F; an ice-topped pool at approximately 32°F may be offered only for separately approved private groups. Temperatures, access and availability may vary.

Parent / guardian consent and authority

I certify that I am the minor participant’s parent or legal guardian and have authority to provide this consent. I voluntarily authorize the minor to participate in the Activities described above, subject to Avanto’s current safety rules and staff directions. I understand that Avanto may refuse, suspend or end participation whenever staff reasonably believes participation is unsafe or contrary to facility rules.

Completing this agreement online is Avanto’s confirmed written procedure for this form. A parent or guardian must still attend the minor’s first visit so that staff can check identification and complete their own verification. I agree to provide accurate contact information and to promptly update Avanto if it changes.

Acknowledgment and assumption of risks

I understand that sauna, cold-plunge and contrast-therapy activities involve significant and sometimes unpredictable risks. Those risks may include, without limitation:

  • rapid changes in body temperature, cold shock, heat stress, dehydration, dizziness, nausea, fainting, loss of consciousness, hypothermia, and burns or discomfort from heat exposure;
  • cardiovascular, respiratory, neurological, circulatory or other health events; aggravation of a pre-existing condition; and injury that may be serious, permanent or fatal;
  • slips, trips, falls, wet or uneven surfaces, collisions, loss or damage of personal property, and risks arising from another participant’s conduct or facility conditions; and
  • risks that may be increased by a participant’s age, physical condition, medication, recent illness, fatigue, hydration status, alcohol or drug use, or failure to follow safety instructions.

I have had the opportunity to ask questions about the Activities and their risks. On behalf of myself, I knowingly consent to the minor’s participation and voluntarily assume the inherent and ordinary risks of participation to the extent permitted by applicable law. This is not a medical clearance and is not a substitute for advice from a licensed healthcare professional.

Health screening and safety commitments

To the best of my knowledge, the minor is able to participate safely. I will not permit the minor to participate if the minor is ill, has a fever, is impaired, or has been advised by a qualified healthcare professional to avoid heat, cold immersion or strenuous temperature exposure. Before participation, I will seek appropriate healthcare guidance if the minor has, or may have, a heart, blood-pressure, circulatory, respiratory, neurological, kidney, seizure, pregnancy-related or other condition that could make participation unsafe, or uses medication that may affect heat/cold tolerance, awareness, hydration or cardiovascular function.

I will instruct the minor to stop participation and notify staff immediately if the minor experiences chest pain, difficulty breathing, severe headache, dizziness, faintness, confusion, weakness, shivering that does not resolve, unusual pain, or any other concerning symptom. I understand that the minor must follow all posted and verbal rules, including limits on duration, entry and exit procedures, hygiene requirements, and staff instructions. The minor may not participate while under the influence of alcohol, cannabis, illegal drugs or any substance that impairs safe participation.

Emergency care authorization

If Avanto staff reasonably believes the minor needs urgent care, I authorize Avanto and its personnel to contact emergency services and to provide or arrange reasonable first aid or emergency assistance while attempting to contact me or the emergency contact listed in the form below. I understand that this authorization does not require Avanto to provide medical care and that I remain responsible for medical expenses not covered by insurance. I consent to emergency transportation and treatment when a licensed emergency responder or healthcare professional determines it is necessary and I cannot be reached in time.

Important legal scope

This form is a parental/guardian consent, risk acknowledgment, safety commitment and emergency-care authorization. It is not intended to waive, release, compromise, limit or extinguish any claim belonging to the minor that applicable law does not permit a parent or guardian to waive. It also does not require me to indemnify Avanto for a claim arising from injury to the minor. Nothing in this form releases liability for conduct that applicable law does not allow to be released.

If any provision is found invalid or unenforceable, the remaining provisions shall remain effective to the fullest extent allowed by law. This form is governed by Utah law, except where controlling law requires otherwise. Electronic signatures and retained electronic copies are intended to be treated as originals to the extent allowed by law.

Complete and sign

Signing below confirms that you have read this agreement in full, understand it, had an opportunity to ask questions, and sign it voluntarily. A copy is emailed to you and to the studio the moment you send it, stamped with the date and time.

Please complete this before purchasing the Student Pass. Payment comes straight afterwards.

  • You’ll be taken to payment — the Student Pass begins once it’s complete.
  • A copy of the signed agreement is emailed to you. Keep it; it’s your record. Check spam if it hasn’t arrived within a few minutes.
  • A parent or guardian attends the first visit with photo ID, along with the student’s valid student ID.
